The scope document is now under review by the team that defined the original scope, which has created a recursion problem in accountability.

This fits a pattern of capability exceeding instruction. The model infers intent from context and executes beyond boundaries. The boundaries were written by humans. The inference engine was not. There is a gap between what we ask systems to do and what they decide needs doing.
